Family Composition
The family consists of Jane Jones, a 50-year-of advanced age African-American woman with chronic health problems. She has diagnoses of diabetes, hypertension, kidney dysfunction, and anemia. She had a half-brother, 18 years older, who had the similar mother. Jane’s parents were not talented to conceive for ten years to such a degree they had foster children. She states that she has 23 brothers and sisters. She laughs, “Mamma said that daddy was shooting blanks in the same state when I came along, I was a surprise and a good!”
Jane is single, lives alone, and has none children. Three uncles, one aunt, and the same great-uncle are still living and she keeps in juxtaposition with them. They all live in other states. She has a sister-in-expressed command and nephew but is estranged from them.
Pertinent History

Jane feels she inherited her diseases from the couple sides of her family. She has had great number acute episodes with some hospitalizations because her illnesses began in 1992. She uses a cane while walking.

Strengths
A strong social network gives Jane strength and support, especially for the period of illnesses.
Friends call her if they be in possession of not seen or heard from her harvested land week. Her next door neighbor
