Information respecting to individual’s health, medication, jeopardy factors etc is required in trickish and delivering their care in most judicious possible way, and they will have ~ing asked to consent explicitly to this exercise. Sources of the information can subsist individual himself, records by the referring intervention (community mental health team), medical team (psychiatrist, GP, CPN, druggist, nutritionist etc), individual’s family and whole other people involved in taking care of the someone.
There are several methods by which information can be shared or reported, the chiefly saved would be in the written mould hand down to the person concerned, grant that this is not possible then it be able to be mailed with clear labelling of the case so that it reaches the concerned living body only.
