1) Application Form

-passport shall contain two struck dumb. pages
-current passport shall be powerful for a period of 90 days at a distance the date of return to Sri Lanka - preceding passports should be submitted at the meeting -lost passports should be declared through a police report
(translated into english people and certified by the Consular Division - Ministry of Foreign Affairs Colombo) - photocopy of passport bio-page and any amendments 4) Covering Letter – self addressed by the applicant explaining the purpose of survey and travel plan…..etc 5) General Study visa :
-“Dichiarazione di Valore” (Declaration of Value) of the suitor’s educational /professional certificates duly legalized through the Consular office of this Embassy. (The University/Institute or School should inform in case it is not required) -Documented guarantees respecting the study course, professional formation or cultural action undertaken. -Adequate financial guarantees regarding the way of sustenance. Not lower than the figures established ~ dint of. the Internal Ministry as per Article 4, comma 3 of the T.U. 268/1998 or a Scholarship from the Italian enactment in this connection. -Acceptance of enrolment ~ dint of. the Italian School or University or Institution, stating time of the course , course structure, advantage, financial grants, ... etc. - receipt of requital of tuition fees
